Vaccination Requirements for Kindergarten
New Immunization Requirements for Ninth Graders

The school health record must include the following information:
The child's name and date of birth
A minimum of:
Five DTP (4 DTP if 4th is given on or after 4th birthday)
Four Polio (3 Polio is 3rd is given on or after 4th birthday)
Two MMR (measles, mumps, rubella)
One Varicella (chicken pox) or a reliable history of disease

Each immunization entry must include:
Vaccine type, date administered and the name of the provider.


Sstudents entering the 9th grade in September 2005 will have a change in immunization requirements. Please be aware that these changes may impact your child.

Your child will be required to have one of the following:

A note from the student's physician stating that your child has had chickenpox.
Documentation that your child has had the varicella (chickenpox) vaccine.
Results of a blood test that shows your child is immune to chickenpox.
A signed parental/guardian waiver form or letter, that indicates your sincere religious belief or philosophical reason for not immunizing your child.
A letter from your child's healthcare provider explaining the medical reason for not getting immunized.

The table below provides information on when the chickenpox vaccine will be required.
